An exciting opportunity has arisen to work within the Veterans Trauma Network nationally across England.

The role of Care Coordinator has been developed specifically to support veterans of the Armed Forces who have service-related physical health needs. This role is part of a pilot and then national roll-out to further develop the Veterans Trauma Network which has been in place since 2016. Initially set up for veterans with trauma-related physical injuries, this post will support the development of the programme and a system of care for veterans that incorporates social prescribing and clinical advice to form a holistic approach to treatment.

Successful candidates will have a good knowledge of the military culture and environment and be clinically qualified. The postholder will oversee the caseloads of Veteran Support Workers as well as developing care plans to enable veterans with service-related physical health needs to lead fulfilling lives. The post holder will assess, plan and implement care, and provide specialist clinical advice and will be expected to work closely with providers on the implementation of a range of performance and quality improvement programmes.

The successful application will be key in promoting the VTN within military charities, veterans mental health services and the wider NHS. The role will involve liaising with GPs, hospital consultants and other stakeholders to ensure the coverage of the VTN is as wide and varied as necessary to support veterans. Due to the nature of the role the post holders must be able to demonstrate strong analytical skills, be comfortable working with senior managers and clinicians across health and social care, have good written and communication abilities, be a self-starter and able to simultaneously manage competing deadlines.
